Preparation for Ministry FORMS

Information can be entered directly into the forms using the free Adobe Acrobat Reader program, and then saved so that they can be emailed to others, updated later, etc. Submit forms with required attachments to Kevin.Nollette@Seattlepresbytery.org.

Preparing for Ministry

The Committee on Preparation for Ministry is responsible to guide the process for full preparation of individuals who are to be ordained as ministers of the Word and Sacrament. For information on how to enter this process, download the Process Protocols.

Assignment to an Advisory Group will be made upon submission of completed application forms 1 and 2A and B. Electronic submission of forms is encouraged when possible. Email to: barbara@presbytery.org . Mail hard copies to:
